I am aware of the Young Mayors program, understand the nature of the activities and authorise my child’s participation in these activities. These activities include travel via Taxi and attending a tour at Dalki Garringa Nursery on Friday, 3rd of May.

I give permission for my child to be photographed and filmed with their consent, and understand that my child’s image, words and story may be reproduced across FYA’s website and social media channels, and the participating Council’s website and social media channels

I understand that the duration of this consent is for the length of my child’s participation in the program, and I can revoke consent at any time by contacting FYA on the details below.
