Melbourne Office Partner Intake Form

We ask for some personal details in this form. This will be used to onboard you as a Melbourne Office Partner.

Part I: Welcome

Our office is located on the unceded lands of the Wurundjeri People of the Kulin Nation at Naarm. We recognise their continuing connection to land, water and community and pay respects to Elders past, present and those yet to come. We acknowledge that sovereignty was never ceded. This always was, always will be Aboriginal Land.

To finish setting you up as a user of the office, please complete this form. If you have any questions, please contact Lucy Nakata at lucy.nakata@fya.org.au Operations Coordinator.

This helps us understand the best way to address each other and respects the wonderful gender diversity in our community. If you'd prefer not to write your pronouns, you can write "prefer only being referred to as my name".
This will help us plan your induction and off boarding.

Part II: Accessibility

Our office is located at Level 1, 136 Exhibition Street, Melbourne, 3000, VIC.

The FYA office is located on the first floor, and is accessed through wheelchair accessible lifts. The lifts do not announce floor numbers or have a hearing loop. 

Entry and exit to the office is via a wheelchair accessible door which opens and closes automatically and is operated via a swipe card.

The office has facilities including:

  • A kitchen with microwave and fridge
  • All gender toilets and shower
  • Storage lockers
  • If you ride to work, there is a space in the office you can bring your bike or scooter

The office has workstations set up with a monitor, mouse, keyboard, WiFi and printing. You will be able to plug your laptop straight into any available docking station (Mac’s require adapters, which are kept in the office).

A parent, prayer and wellbeing room is available for use.

Information about First Aid Officers and Fire Wardens is displayed in the office.

First aid and health and safety information is posted around the office.

 

Please note: Currently all office usage is provided completely free, without any charge

In the future, FYA may charge a fee for the use of this office space to defray running costs.

All users will be informed well in advance of any changes.

Part III: Terms and Conditions

To ensure that our space is inclusive and safe for everyone, FYA has Terms & Conditions for the office that all users must follow, outlined below:

  1. While using the office you must follow all health and safety directives given by relevant FYA staff members or building management.
  2. Please do not use the office if you are feeling unwell. COVID-19 RAT tests, masks and hand sanitiser are available for use.
  3. While using the office you must abide by relevant FYA’s policies, including our Privacy Policy, Whistleblower Policy and Child Safety and Wellbeing Policy.
  4. Please respect the space and the people within it.
  5. FYA may, at its sole discretion, decide to terminate your use of the office at any time.

For young people across Australia who are facing injustice and are locked out of power, FYA is an independent organisation that backs them to create change. We use our expertise, resources, and connections to back young people – on their terms.
